Σκοπός: Σκοπός της παρούσας εργασίας είναι η διερεύνηση της αποτελεσματικότητας φωτοθεραπείας ως μονοθεραπεία στην πρόληψη και διαχείριση του παραληρήματος στους νοσηλευόμενους ασθενείς στις ΜΕΘ. Υλικό και Μέθοδος: Η αναζήτηση της σχετικής βιβλιογραφίας πραγματοποιήθηκε στις βάσεις δεδομένων PUBMED, COHRANE LIBRARY και CINAHL με τις λέξεις κλειδιά: delirium, prevention, critical ill, light therapy, bright light therapy, circadian rhythm και τους συντελεστές Boolean “AND” και “OR” ανάμεσα τους. Αποτελέσματα: Από την αναζήτηση προέκυψαν 6 μελέτες για ανασκόπηση που πληρούσαν τα προκαθορισμένα κριτήρια, κατά την περίοδο 2007-2021. Μέσα από την αναζήτηση της βιβλιογραφίας διαπιστώθηκε πως ο κιρκάδιος ρυθμός επηρεάζεται αρκετά κατά την παραμονή των ασθενών στις ΜΕΘ. Οι έξι μελέτες που μελετήθηκαν ήταν ποσοτικές και είχαν πραγματοποιηθεί σε μονάδες εντατικής θεραπείας γενικού τύπου ή μετεγχειρητικές μονάδες εντατικής θεραπείας. Η διερεύνηση της αποτελεσματικότητας την φωτοθεραπείας, σαν μη φαρμακευτική παρέμβαση, για την αντιμετώπιση της επίπτωσης του παραληρήματος βρέθηκε να έχει στατιστικά σημαντική συσχέτιση σε μόνο 2 από τις 6 μελέτες που διερευνήθηκαν. Συμπεράσματα: Η φωτοθεραπεία σαν μονοθεραπεία για την αντιμετώπιση του παραληρήματος στις ΜΕΘ δεν είναι πάντα αποτελεσματική και δεν επιφέρει μείωση της επίπτωσης ή της διάρκειας του. Παρόλα αυτά, άλλες μελέτες έδειξαν ότι η φωτοθεραπεία σαν μονοθεραπεία σε δομές εκτός ΜΕΘ και σαν μέρος πολυσύνθετων μη φαρμακευτικών παρεμβάσεων μπορεί να είναι αποτελεσματική. | Description: | Introduction: Delirium is a common occurrence in hospitalized patients, particularly those in intensive care units. Aim: The objective of the current thesis pertains to the examination of the efficiency of phototherapy as potential monotherapy for the prevention and management of delirium in ICU patients. Material and Methodology: The relevant literature search was conducted in the PUBMED, COHRANE LIBRARY and CINAHL databases using the keywords delirium, prevention, critical ill, light therapy, bright light therapy, circadian rhythm as well as the “AND”, “OR” Boolean operators between them. Results: The aforementioned search produced 6 review studies within the 2007-2021 time frame, that fulfilled the predefined criteria. The thorough investigation of the pertinent literature revealed that the circadian rhythm is affected appreciably during the patients’ hospitalization in ICU. The six review studies examined were quantitative and were conducted in general intensive care facilities or post-operative intensive care units. The investigation of the potential effectiveness of phototherapy, as a non-pharmaceutical treatment for the mitigation of the adverse effects of delirium, has been statistically supported in the findings of only two of the six published studies examined. Conclusions: Phototherapy, as a monotherapy regimen, is not always effective for the treatment of delirium and does not usually result in significant reduction of its probability of onset or its duration.
